Caring for Our Community’s Health
South Florida is where we work and live. This vibrant, diverse community means the world to us. At Memorial Primary Care, we’re dedicated to improving the health and well-being of the people who call South Florida home.
As primary care providers, we understand the impact nonmedical factors can have on our patients. We see that reality as we care for people throughout their adult lives.
We screen for conditions that influence the lives of our patients beyond health care services, including housing, employment, food insecurity, transportation, safety and more. These screenings help us connect patients with our many community partners who can help support their needs.
Bringing Care to the Community
The health and wellness options at Memorial Primary Care go far beyond the walls of our standard locations. We bring a range of health programs and services into the community, such as:
- Adult and Pediatric Mobile Primary Care Health Center: Through our “One City at a Time” program, our adult mobile health and pediatric mobile health clinic buses are stationed in different cities in South Broward County for extended periods of time. Services include physical exams, sick visits, school physicals, vaccines and nutritional counseling. No appointment is needed and there is no charge — but there are eligibility requirements.
- Breast imaging for women of color: We know that breast cancer mortality rates are disproportionately high for Black women and other women of color. To help overcome this disparity, our adult mobile health clinic offers free breast cancer screenings to minority women in underserved areas of South Broward county.
- OB nurse navigators for women of color: This program monitors expecting and new mothers for high blood pressure from their homes. OB nurse navigators make sure moms know when to go to the emergency room and help them get primary care for themselves after giving birth.
- Sickle Cell Day Center: People with sickle cell disease often need extra support and care. This day program provides treatment, health coaches, social workers and other support staff. People can get all the care they need in one place and reduce their risk of hospital admissions. Get more information on sickle cell disease treatment.
Outreach and Education Programs
Our community-based initiatives include health education, disease prevention and outreach and advocacy efforts:
- Cancer education series (I Can Cope): These classes feature guest speakers who teach patients and families about various aspects of cancer. Topics may include diagnosis, treatment, nutrition issues and more. Call 954-265-5768 for more information.
- Health fairs: We’re out in the community regularly to promote health and wellness, offer screenings and share knowledge and advice.
- Social and wellness activities for older adults: Memorial Senior Partners offers exercise and dance classes, health screenings, lectures and more.
