What is Connected Care?
Connected Care is Memorial Healthcare System's remote patient monitoring program. By using our monitoring devices at home, you can easily track important health information, including:
- blood pressure,
- weight,
- temperature, and
- blood oxygen levels.
Our remote patient monitoring devices automatically send your measurements to your healthcare provider, who can help you keep an eye on any changes to your health.
- Connected Care is not a replacement for emergency or urgent care.
- If you’re experiencing a medical emergency, call 911.
Who can participate in Connected Care?
You can enroll in Connected Care if you are:
- A patient at Memorial Primary Care
- A patient in the surrounding community seeing a provider outside Memorial Healthcare System
- An employee at Memorial Healthcare System*

How does Connected Care work?
After you enroll, your doctor will help you decide which devices to use. Then, we’ll send the healthcare monitoring devices directly to your home and help you get them set up.
You’ll also receive our SmartHub to connect your monitoring device(s). The SmartHub device gathers all the measurements you collect and sends them to your doctor. Your measurements are automatically sent to your MyChart.
We currently provide the following remote patient monitoring devices.
Blood pressure cuff
Measures blood pressure, which can be an indicator of heart disease, stroke and other conditions.
No-touch thermometer
Measures your body temperature. An elevated body temperature could be a sign of infection.
Scale
Measures body mass and weight. Weight gain or loss can be important to track when managing conditions like diabetes.
Spirometer
Measures how much and how quickly you can inhale and exhale air. It can help monitor conditions like chronic obstructive pulmonary disease (COPD) and track how you respond to treatment for lung conditions.
Glucose monitor
Measures glucose levels which, if too high, can lead to diabetes. Your doctor can track trends, including spikes in blood glucose levels.
Pulse oximeter
Measures how much oxygen is in your blood. It can help make sure your lungs are working properly.

It’s convenient
All you have to do is record your measurements — we’ll take care of the rest. Your doctor reviews your data, and if there are any questions or concerns about your measurements, your healthcare provider will contact you.
You can also record your measurements wherever you are. The SmartHub doesn’t require WiFi, just an outlet for power. That means you can record measurements even if you’re on vacation.
We support you
While you’re enrolled in Connected Care, we’re with you every step of the way. We’ll help you set up your devices, answer any questions about them and check in with you periodically while you’re in the program. You can also reach out to your healthcare provider anytime with questions.
You stay proactive
Monitoring key health measurements for a chronic condition like diabetes or hypertension can improve your overall health and quality of life. Regular updates from Connected Care allow your doctor to recommend more timely treatments, keeping you out of the hospital and preventing serious health complications.
