Memorial Healthcare System's New Care Coordination Center Will Enhance Patient Safety and Care Efficiency

Memorial Healthcare System has taken another step to create a better patient experience with the launch of its new Care Coordination Center (CCC). The CCC is a 24/7 clinical command center in Pembroke Pines that uses virtual operations and technology to improve patient safety and care times across all of Memorial’s healthcare facilities. It will accelerate patient transfers and adjust staff needs in real time.

“At Memorial Healthcare System, we are dedicated to enhancing patient care through innovative solutions. The Care Coordination Center allows us to leverage real-time data and collaborative approaches to improve patient safety and streamline care delivery,” said Elizabeth Justen, chair, South Broward Hospital District Board of Commissioners. “This initiative ensures that we provide the best possible experience for our patients while improving operational efficiency.”

The CCC uses technology and dashboards to provide real-time views of patient bed use, staffing needs, and virtual monitoring. With 3,000 square feet of workspace and 31 workstations using AI-driven technology, the CCC helps ensure patients are receiving the best care by reducing wait times for Emergency Department patients needing admission, decreasing patient falls, and improving admission and discharge efficiencies.

The Care Coordination Center has several features that are used to enhance patient satisfaction:

  • Transfer Center Coordination: The CCC streamlines patient transfers from the Emergency Department and to/from other hospitals, ensuring fast admissions that lead to quicker treatment and care.
  • Virtual Patient Observation: With the goal of reducing patient falls, a dedicated team will virtually monitor patients who are at high risk for falls, ensuring immediate help.
  • Virtual Nursing Initiative: The Virtual Nursing program enables nurses to virtually handle admissions, discharges, and patient education, freeing bedside nurses to focus on direct patient care.
  • Centralized Staffing: The CCC assesses nursing and other demands for staff, moving caregivers to the appropriate patient locations as needed.
  • Centralized Bed Placement & Capacity Management: Dedicated staff oversee patient bed assignments to maximize capacity and improve patient transitions.
